Provider/Community Survey
Please choose this version of the survey if you do not receive direct services, but are a service provider, funder, or would like to have your voice heard as a concerned member of our communities. If both surveys are applicable to you and you are willing to fill out both, please feel free to do so.
What is a Community Needs Assessment?
Who can take the survey?
Anyone who is a part of the Lewis, Mason, or Thurston County communities! You do not need to be a client of CACLMT. If you care about your community, we want to hear from you!
Can I forward the survey to other people?
Yes! Please feel free to share the survey with others who live or work in Lewis, Mason, or Thurston County. The more voices we hear, the better we can understand community needs and create programs that truly reflect them. Thank you for helping to spread the word!
Can I complete the survey more than once?
No, please only take the survey once. Multiple individuals that live in the same household may take the survey, but please only complete one survey of the same survey type (Provider or Community Member) per individual.
Should I fill out the Provider survey or the Community Member survey?
Please take the survey that feels most reflective of your connection to Lewis, Mason, or Thurston County. If you provide services to Lewis, Mason, or Thurston County community members, please complete the Provider survey. If you or your family receives services, either from CACLMT or other agencies and organizations within Lewis, Mason, or Thurston County, please complete the Community Member survey. If you provide services to others and also receive or have received services yourself (or your family), then you may complete both surveys if you would like to. (it is optional to complete both)
If I live and work in Lewis, Mason, or Thurston County should I fill out both the Provider survey and the Community Member survey?
The provider survey and the community member survey generally ask different questions. If you both work within the community and receive services within the community, you are welcome to complete both surveys. (Completing both surveys is optional.)
How long will the survey take to complete?
The survey will take approximately 15 minutes to complete.
How will my survey response information be stored?
Your privacy is important to us. The Community Needs Assessment is anonymous and designed to protect your privacy. Responses are collected through Microsoft Forms, which uses Microsoft’s secure cloud systems. No names or personal identifiers are required. Once you submit your survey, the data is stored on our private cloud server, an encrypted and secure platform. Access is limited to authorized staff, and individual responses will never be shared publicly.
When will I be able to see the results of the survey?
A Community Needs Assessment Report that highlights CACLMT projects from the last three years and details Key Findings from the 2026 CNA Survey will be published and publicly available in Spring 2026.
What happens after the results of the survey?
Once the survey results are gathered, they will guide the development of the 2026 Community Needs Assessment Report, a resource built to strengthen our communities. This report will not only showcase CACLMT’s projects from the past three years but also spotlight the voices and priorities shared through the survey. By making the report publicly available, we aim to empower community members, local organizations, and partners with insights that drive meaningful changes and ensure programs truly reflect the needs and voices of the people we serve.
Is there a chance I’ll win a $25.00 Fred Meyer Gift Card?
Once you have completed the survey you have an option to provide your contact information for a chance to win a $25.00 Fred Meyer gift card! Community Member survey only. One gift card per individual. Drawings will take place at the end of January 2026. CACLMT employees are not eligible to win.
